Contract for counselling with Jim Friedman
The aim(s) of your counselling with me
We will talk together in order to address the concerns
discussed and agreed at your Consultation interview and any others that emerge
as counselling continues. We will explore and try to understand those concerns
and, where appropriate, discuss how to bring about changes that are relevant
to you and your relationships.
Confidentiality
Whatever you discuss in your consultation or counselling
sessions with me is confidential, unless I believe that either you or a third
party is at risk. If this is so, then, where appropriate, I will discuss with
you how I would like to proceed with your co-operation.
Notes of sessions are kept separately from contact
details in order to protect your confidentiality.
My work with clients is discussed, usually anonymously,
with my supervisor as part of my professional accountability. If I need to
consult with another professional or a third party I will ask for your written
permission to do so.
Payment
Payment for each consultation or counselling session will
be made by cheque (supported by a banker's card) or in cash. The payment for
each session will be £……
Cancellation
In order to offer an ethically-minded service supported
by professional training, maintenance of accreditation and regular supervision,
I rely on prompt client payments.
If you need to cancel a session, please notify me as much
in advance as you can. If less than two working days notice is given, the
missed session will need to be paid for.
Client feedback
I welcome comments on your experience of counselling with
me. If you would like to do so, please offer your feedback either in person
in the session itself or by letter or phone, whichever is preferable to you.
You will be invited to complete a questionnaire at the close of your counselling
with me.
I work according to the Ethical Framework of
the British Association for Counselling and Psychotherapy, 15 St John's Business
Park, Lutterworth LE17 4HB
I/We have read this statement and understood its terms.
